Community Development Worker - Healthy Communities
We are seeking to appoint a Community Development Worker: Healthy Communities Together - Leeds (18.5 hours).
In Leeds the Solidarity Network, with our NHS and local authority partners, have been awarded funds to deliver phase two of Healthy Communities Together (HCT). HCT is an exciting funding strand from the National Lottery Community Fund in collaboration with the Kings Fund; supporting systems change in health care services through VCS, NHS and local authority working together. We aim to move from transactional to mutual partnerships to impact and benefit marginalised communities of Leeds and improve poor health outcomes. The marginalised communities this project aims to ensure are involved in their health care are Gypsies and Travellers, Sex Workers, Asylum Seekers and Refugees and Trans+ communities. The Solidarity Network are: Leeds GATE, BASIS Yorkshire, LASSN and Yorkshire MESMAC.
Aims-
- Support the Systems Change Coordinator in delivering Leeds HCT project activities, keeping the needs of marginalised communities central.
- Facilitate coordination among the Solidarity Network partners and with NHS and Local Authority stakeholders.
- Support Systems Change Coordinator with participation, communications, and reporting for the project.
- Work with our partners to identify future community leaders and engagement in the project.
- Contribute to reducing health inequalities experienced by marginalised communities in Leeds.
- Support Systems Change Coordinator with administrative responsibilities
Responsibilities & Duties.
- Involve, train, supervise, and support volunteers and community leaders, particularly those with lived experiences from marginalised communities.
- Work alongside creative providers (e.g., graphic artists, storytellers, filmmakers) to document learning and progress of the HCT project.
- Support the Systems Change Coordinator with offering our programme of facilitated spaces, training, and learning events to healthcare professionals in Leeds, in collaboration with the Solidarity Network, NHS, and Local Authority partners.
- Liaise with other service providers (e.g., housing services, Gender Identity Clinics, drug projects) to develop joint initiatives that benefit the HCT project outcomes.
- Alongside the Systems Change Coordinator, support monitoring and evaluating the work delivered by Yorkshire MESMAC and the Solidarity Network under the HCT project.
- Support HCT Coordinator with minuting meetings and correspondence.
- Support Systems Change Coordinator with producing reports for the HCT project, including those required by The King’s Fund, The National Lottery, The Solidarity Network, and other partners.
- Champion the values of the Solidarity Network in all aspects of work.
Salary: NJC SCP 18 - £29,269 pro rata per annum. Contract is fixed to 30th June 2025. Salary is reviewed annually. There is a 5% workplace pension (on completion of probationary period). This post is subject to an Enhanced DBS with barred lists check.
